Schedule: Every other weekend required; weekday caseload up to 15 patients, higher on weekends based on census

• Comprehensive case management role including utilization review, care coordination, and discharge planning
• Epic charting system utilized throughout the facility

• Interdisciplinary team support including interpretation services and social services
• Serve diverse patient populations including adults and geriatrics

Requirements:

• Active California RN license (pending licenses not accepted)
• Minimum 2 years of acute care experience required (Medical-Surgical, MS/Telemetry, ICU, and some Emergency Department experience mandatory)
• Previous case management experience in California required
• Proficiency with Epic EMR system required
• Expertise in InterQual criteria, care coordination, utilization review, and discharge planning required
• Community hospital experience required
• Previous travel nursing experience required (first-time travelers not accepted)
• Strong skills in medical necessity determination, concurrent review, admission criteria, and continued stay reviews
• Knowledge of CMS guidelines, DRG, ICD-10 coding, and HIPAA compliance
• Ability to commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Maximum 7 days of time-off requests during assignment
