Schedule: Full-time with weekend rotation; Float capability to nearby facility required; 40-hour orientation provided

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years acute medical Care Manager experience performing care coordination
• Previous travel healthcare experience required
• Epic charting system proficiency required
• Trauma Level I or II experience preferred
• Strong skills in assessing patients, discharge planning, and family meeting coordination
• Experience with HMOs, IPAs, and various insurance frameworks
• Knowledge of community resources including foster care, substance abuse treatment, mental health services, and homelessness support
• Competency in arranging DME, SNF placement, and ambulance transport independently
• Understanding of appeals, denials, DRG classification, and CMS guidelines
• Familiarity with HIPAA, OSHA, and Joint Commission standards
