Schedule: Every other weekend required; caseload up to 25 patients; no call required; floating based on department needs

Requirements:

• Minimum 2 years of acute care hospital case management experience, current within the last year
• Epic charting system experience required
• Experience with HMOs, IPAs, and managed care environments
• Knowledge of advance directives, POLST, and protective services processes
• Proficiency in admission criteria, discharge planning, DRG, appeals and denials, and prior authorizations
• Active RN license and ability to provide 2 professional references covering 1 year within the last 3 years
• Willingness to float between units based on departmental needs
