Description
Start Date: June 1, 2026
Location: Kansas City, Missouri
Compensation: Details provided to qualified applicants
Schedule: Monday through Friday, 8:00 AM – 4:30 PM with rotating weekends
Highlights:
- Acute care case management role in a 590-bed medical center
- Patient ratios of approximately 1:25, varying by census and acuity
- Collaborative team environment with RNs, Social Workers, and Case Management Assistants
- Documentation systems include Meditech, Midas, and Wellsky
- No on-call requirements

Requirements:
- Active RN license in Missouri or compact state eligibility
- Recent acute care discharge planning experience required
- Proficiency with electronic medical records and case management platforms
- Flexibility to float to other units as needed
