Schedule: Monday through Friday, 8:00 AM to 5:00 PM

Highlights:

• Coordinate comprehensive patient care management with a focus on resource optimization and quality outcomes
• Work collaboratively with interdisciplinary teams (IDT) to ensure continuity of care and patient satisfaction
• Utilize MCG criteria for utilization review and discharge planning across acute care settings

• Serve as patient advocate while managing complex cases in an acute care environment

Requirements:

• Active Texas RN license or multi-state RN license
• Current BLS certification through the American Heart Association
• Minimum 2 years of recent RN Case Management experience
• Experience in LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) settings required
• Demonstrated proficiency with MCG (Milliman Care Guidelines)
• Strong background in discharge planning across acute care environments
• Proven experience with interdisciplinary team collaboration
