Hours guaranteed: 36 and 85% productivity standard

Schedule: M-F

Days per week: 5

Setting: SNF

Volumes per day: 6.5 hours of treatment

Location: Westwood Specialty Care - primary; back up Sunrise Retirement Community and Accura Sioux City
