Description
BAYADA Home Health Care is seeking a Per Diem Registered Nurse (RN) to join the Fort Collins Visits team and work with adult and senior clients on a per visit basis.
Please note: current or recent experience in a Medicare Certified Home Health setting with OASIS documentation experience is required for a per diem position.
As a home health nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
How You’ll Make an Impact:
- Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
- Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
- Triage and manage urgent clinical situations independently in the home setting
- Perform medication reconciliation, teaching, and safety monitoring
- Perform comprehensive wound assessment, care, and documentation

Qualifications for a BAYADA nurse:
• A current Colorado RN license in good standing
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
Additional Information:
- Pay: $55-60 per point, based on qualifications
- Mileage reimbursement
