Job Details

• Position: Home Health Physical Therapist (PT) – Travel / Contract
• Location: On-site in Lancaster, OH (43130) — Lancaster, OH home health job, Fairfield County, Ohio
• Contract Length: 13 weeks
• Start Date: April 6, 2026
• Shift: Days (5 shifts per week, 8 hours per shift)
• Hours per Week: 40 (32 guaranteed)
• Estimated Weekly Pay: $2,569 - $2,854

PT Job Requirements

• Minimum 1 year of experience as a licensed Physical Therapist (PT)
• At least 6 months of Home Health experience as a Physical Therapist
• Experience with OASIS and HCHB required for clinical documentation
• Current, active Physical Therapist license in the state of Ohio
• Current CPR/BLS certification
• Valid driver’s license, vehicle insurance, and reliable vehicle or access to public transportation for in-home visits

Home Health PT Responsibilities

• Provide high-quality in-home physical therapy services focused on mobility, independence, fall prevention, and functional goals
• Perform comprehensive patient assessments and develop individualized treatment and discharge plans
• Document assessments, treatment plans, progress notes, and OASIS data using HCHB to ensure accurate clinical records
• Coordinate care with interdisciplinary teams, physicians, nurses, patients, families, and caregivers
• Educate patients and caregivers on therapeutic exercises, use of adaptive equipment, safety strategies, and home modifications
• Maintain professional standards, comply with regulatory requirements, and ensure timely, accurate documentation for home health compliance
