- Schedule: Monday – Friday, 8:00 AM – 4:30 PM (8-hour days)

Job Requirements / Qualifications
- Minimum 5 years of recent RN case management experience, including LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) settings
- Proven experience with MCG guidelines, utilization review, and discharge planning across acute care settings
- Active Texas RN license or compact/multi-state RN license in good standing
- Current BLS certification through the American Heart Association (AHA)
- Strong interdisciplinary team (IDT) collaboration, communication, organizational, and patient advocacy skills
- Case management certification (CCM) or related credential preferred but not required
Primary Responsibilities
- Coordinate and manage patient care to optimize resource utilization and ensure compliance with regulatory and external review agencies
- Conduct comprehensive assessments, develop individualized care plans, implement interventions, and evaluate outcomes
- Integrate case management, utilization review, and discharge planning functions to support safe, timely transitions of care
- Serve as a patient advocate, addressing holistic needs and facilitating appropriate community and facility resources
- Collaborate with physicians, nurses, social workers, therapists, and other IDT members to align care with patient preferences and goals
- Document case management activities, clinical decisions, and utilization review outcomes according to facility and payer standards
