Travel Home Health Registered Nurse (RN) – Newton, MA | 13-week assignment | 1+ year home care & case management experience required. Join our Newton, Massachusetts homecare team delivering patient-centered home health nursing across the South Region.

Job Details

• Location: Newton, MA (02459) and surrounding towns – serving Ashland, Fayville, Framingham, Hopkinton, Hudson, Marlboro, Southboro, Sudbury, Waltham, Wayland, Weston and the Greater Boston area
• Assignment Length: 13 weeks (May 31, 2026 – August 29, 2026)
• Weekly Hours: 40 guaranteed
• Shift: 8:00 AM – 4:30 PM (8-hour day shifts)
• Employment Type: Travel / Home Health RN
• Estimated Weekly Pay: $2,500 – $2,630

Qualifications / Job Requirements

• Current Registered Nurse (RN) licensure in Massachusetts
• Minimum 1 year certified home care nursing experience (home health RN)
• Minimum 1 year case management experience (home health case manager)
• Reliable, independent transportation for home visits across Newton and surrounding communities
• Willingness to work weekends, holidays, and participate in on-call rotation (weekend rotation once every 6 weeks)
• Proficient in timely electronic documentation (EMR) and effective interdisciplinary communication

Key Responsibilities

• Provide comprehensive clinical care to patients of all ages in their homes—newborn to geriatric—delivering high-quality home health nursing
• Perform thorough patient assessments and develop, implement, and revise individualized care plans
• Provide patient and family education, disease management, and discharge planning support
• Manage admissions, coordinate services, and carry out case management duties per agency policies and regulatory standards
• Maintain accurate, timely electronic clinical documentation and communicate effectively with physicians, therapists, and interdisciplinary teams
• Travel safely to assigned client homes in Newton and neighboring towns as part of the South Region home health caseload
