• Shift: 8:00 AM – 4:30 PM (8-hour shifts)
• Hours per Week: 40 (Weekly hour guarantee: 36)
• Estimated Weekly Pay: $2,030 – $2,190
• Dress Code: Gray or Black scrubs
• On Call/Weekend Requirement: None
• Mileage Reimbursement: Yes, reimbursed between visits (from first patient to last patient)

Job Requirements / Qualifications

• Active Physical Therapist license (state-specific license required; Ohio PT license preferred)
• Doctoral degree in Physical Therapy (DPT) or equivalent
• Minimum 1 year of recent clinical experience as a Physical Therapist (home health or hospice experience preferred)
• Strong communication, documentation, and organizational skills
• Ability to manage an independent home health caseload and travel between patient homes (reliable transportation required)
• Familiarity with Medicare documentation, home health coding, and regulatory requirements a plus

Responsibilities — Home Health / Hospice Physical Therapist

• Deliver high-quality physical therapy services in patient homes and hospice settings, focusing on functional mobility, pain management, and rehabilitation
• Perform patient evaluations, develop individualized treatment plans, and implement therapeutic interventions to improve mobility and independence
• Provide skilled rehab services including gait training, balance interventions, therapeutic exercises, and ADL retraining
• Collaborate with interdisciplinary team members (nurses, OT, SLP, social work, case managers) to coordinate patient-centered care
• Document patient progress accurately and timely in the electronic medical record, maintaining compliance with company and Medicare standards
• Educate patients and families on home exercise programs, safety modifications, and fall prevention
• Meet productivity expectations while maintaining clinical quality and a positive team environment
