Homecare Registered Nurse (RN) travel assignment in Ontario, OH — Active Ohio RN license, 1+ year Home Health experience, 12-week travel contract. Job Details

• Weekly Estimated Pay: $2,120 – $2,230
• Location: Ontario, OH 44906 and surrounding counties (Richland, Ashland, Huron, Crawford, Marion, Morrow, Knox, Delaware) — North Central Ohio home health jobs
• Assignment Type: Travel Contract / Travel Nursing Assignment
• Duration: 12 weeks (with potential for extension)
• Schedule: Monday – Friday, 8:00 AM – 4:30 PM
• Guaranteed Hours: 40 hours per week
• Shift Length: 8-hour shifts
• On Call/Weekend Requirement: 1 week and 1 weekend during the assignment
• Dress Code: Black and Grey scrubs

Job Requirements / Qualifications

• Active Registered Nurse (RN) licensure in the state of Ohio
• Minimum of 1 year of experience in Home Health nursing or comparable homecare RN role
• Associate's Degree in Nursing or higher
• Familiarity with Homecare Homebase (HCHB) is a plus — experience with electronic home health documentation preferred
• Excellent communication, organizational, and time management skills for home health and travel nursing assignments
• Reliable transportation for travel between patient visits (mileage reimbursed between visits, not to/from home)

Home Health RN Responsibilities

• Provide skilled nursing care to patients in their homes across multiple counties (home health nurse duties)
• Conduct comprehensive patient assessments and develop individualized homecare nursing care plans
• Administer medications, treatments, and wound care as prescribed
• Educate patients and families on disease management, medication administration, and self-care techniques
• Document all care provided in accordance with agency, regulatory standards, and electronic charting systems
• Collaborate with interdisciplinary team members, case managers, and referring providers to ensure optimal patient outcomes
• Participate in on-call rotation and provide occasional weekend coverage as scheduled
