Key Requirements and Credentials

Certifications (Required or Preferred)

• BLS (AHA or ARC) – Required
• PALS (AHA or ARC) – Optional
• ACLS (AHA or ARC) – Optional

Education

• Diploma, degree, or transcripts acceptable
• Board certification and primary source verification of highest education level required prior to first assignment and upon requalification

Licenses

• Valid professional license with primary source verification (e.g., Nursys) within 30 days of start, updated upon expiration
• Driver's License or State ID required

Health and Immunization Requirements

• COVID-19 Vaccine: Proof of vaccine and 1 booster or approved exemption (optional)
• MMR: Proof of 2 immunizations or positive IGG titer; booster or declination accepted
• Hepatitis B: Proof of 3 immunizations and positive antibody titer; booster or declination accepted
• Influenza Vaccine: Required seasonally (Oct-Mar) unless exempted (optional)
• TB Test: Negative test within 30 days of start, updated annually; if positive, chest x-ray and annual questionnaire required
• Varicella: Proof of 2 immunizations, positive titer, or declination accepted
• Tdap: Proof of vaccination within past 10 years or declination accepted
• Drug Screen: Standard 10 panel within 30 days of start, updated annually
• Fit Test: Completed onsite on start date (optional)
• COVID Test: Required on start date for clinical staff (optional)
• Health Statement: Signed statement within 1 year of start confirming ability to work without restrictions, updated annually

Professional and Background Checks

• Criminal Background Check: 7-year county and national check, SSN trace, employment verification (5 years), OFAC, NSO; must be within 30 days of start and updated every 3 years
• OIG Search: Manual search within 30 days of start, updated annually
• EPLS (Excluded Parties List System): Manual search within 30 days of start, updated annually
• OFAC (Office of Foreign Assets Control): Manual search within 30 days of start, updated annually
• National Sex Offender Registry: Manual search within 30 days of start, updated annually
• Medi-Cal Suspended and Ineligible Provider List: Checked within 30 days prior to assignment and monthly thereafter
• Social Security Trace: Optional if not included with background check, must be updated annually
• Employment Verification: Optional if not included with background check, covering past 5 years

Documentation and Compliance

• Resume/Profile Submission required
• 2 Professional References (minimum 1 supervisory) required
• Skills Checklist: Clinical skill-specific checklist required within 1 year of start
• Confirmation of Assignment/Job Description: Required
• Acknowledgement of Receipt for Policy Packet: Required
• Dispute Resolution Agreement: Required
• Badge Photo: Neutral background, forward facing, no facial jewelry
• Release of Information (ROI) for MyChart: Required
• Confidentiality Agreement: Required
• Do Not Rehire/Send (DNS) Attestation: Required from agency on letterhead

Additional Notes

• HLC Testing to be completed by Friday before start date (optional)
• Some immunizations and tests allow for declination with proper documentation
• All credentials and verifications must be current and compliant prior to onboarding
