Licenses & Certifications
- Nursys Verification: Must be completed within 30 days prior to start. Includes FACIS Questionnaire if disciplinary actions exist.
- License/Certificate Verification: Current license and primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
- BLS Certification: Must be American Heart Association (AHA) certified.
- NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
- MOAB Moderate Risk: MOAB De-Escalation Training required.
- Driver’s License/State ID: Must be current, not expired, and match address on file.
- Ascension Profile & Resume: Required for submission.
- Two Professional References: Required prior to first assignment.
Health & Immunizations
- MMR: Documented proof of two vaccines or positive IGG titer. No declinations accepted.
- Varicella: Proof of two vaccines, immunity, or verified history. No declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Declinations accepted with mask requirement.
- 2 Step PPD or IGRA: Required with specific timing before start; TB questionnaire updated annually.
- Positive PPD: Negative chest X-ray within one year and annual TB questionnaire required. (Optional)
- Fit Test: Required for all travelers; preferred before start, can be done onsite.
Testing & Competencies
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics. Completed within one year and updated yearly. Minimum 80% score, max two attempts.
- Ascension EKG Exam: Must pass with at least 80%, max two attempts. Updated exam version required.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
- Ascension Skills Checklist: Clinical skills checklist to be completed within one year and updated annually.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start and updated annually. Includes 12 specific drugs tested. Retesting required if break in service > 90 days.
Onboarding & Documentation
- Signed Assignment Agreement: Required for onboarding (offer letter, initial confirmation, and extensions).
- Ascension Resume: Required for submission.
- Ascension Onboarding Requirements Education Attestation: Completion of pre-hire 6-hour module with attestation.
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
- Ascension Employment Verification: Verification of employment for past 7 years, updated annually or after break in service > 90 days.
- Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check, updated annually or after break in service > 90 days.
- Ascension EPLS: Must be run within 90 days of start and rerun if break in service > 90 days.
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
- Ascension Color Vision: Completed prior to start and updated every 3 years.
Optional
- Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
- Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended if applicable.
- Hiring Manager Evaluation: Post-assignment evaluation.
- Ascension Name Change Document: Legal documentation if applicable.
