Job Description
- Position in Medical/Surgical nursing specialty.
- Facility is part of a large hospital system.
Required Credentials & Certifications
Certifications
- BLS (American Heart Association) - Must be current.
- NIH Certification - Updated yearly or every 2 years depending on group.
- MOAB Moderate Risk De-Escalation Training.
Licenses
- Current nursing license with primary source verification within 30 days prior to assignment start.
- Nursys verification within 30 days prior to start.
- License/Certificate verification required.
Personal Health History / Immunizations
- Ascension Color Vision test (completed prior to start, updated every 3 years).
- MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted.
- Hepatitis B: Vaccination series completion or signed declination.
- Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); declinations accepted with mask requirement.
- Positive PPD or IGRA with negative chest X-ray and annual TB questionnaire.
- 2 Step PPD or IGRA within 90 days prior to start.
- Varicella: Proof of immunity or two doses of vaccine; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Fit Test: Required annually, preferred prior to start.
- Health Statement or physical exam prior to first assignment, updated annually.
Testing & Checklists
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics) - Completed within a year, updated yearly, minimum 80% score.
- Ascension EKG Exam - Passing score of at least 80%, two attempts allowed.
- Ascension Cerner Test-Out - Required.
- Ascension Skills Checklist - Clinical skills checklist, updated annually.
- Ascension 12 Panel Drug Screen - Completed within 30 days prior to start, updated annually.
- Ascension Unit Specific Checklist - Optional for onboarding but must be uploaded within a week if completed.
Professional Requirements
- Ascension Onboarding Requirements Education Attestation.
- Ascension Nursing Contract Worker Onboarding (done onsite).
- Ascension Nova StatStrip Pre-Hire Modules.
- Ascension Badge Photo (forward-facing headshot with white or blue background).
- Driver's License/State ID - Must be current and match address requirements.
- Ascension Profile (resume or professional profile).
- At least two professional references prior to first assignment.
- Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry - Completed within 30 days prior to first assignment, updated annually.
- EPLS (Excluded Parties List System) check within 90 days prior to start.
- Education Verification relevant to clinical work within 90 days prior to start.
- Employment Verification for the past seven years.
- Confirmation of Assignment documentation on agency letterhead.
Additional Notes
- Nursys and license verifications require completion of FACIS Questionnaire if any disciplinary actions exist.
- TB Questionnaire must be updated annually.
- Flu vaccine declinations require mask wearing at all times during flu season.
- Some documents and tests must be uploaded or completed prior to or on the first day of assignment.
- Breaks in service longer than 90 days require re-verification or re-testing of certain credentials.
This position requires thorough compliance with hospital system onboarding, credentialing, and health requirements to ensure patient safety and regulatory adherence.
