Job Opportunity: Care Coordinator

Location: Sacramento, California
Contract Length: 13 weeks


Requirements

  • Specialty: Care Coordinator
  • Years of Experience: Not explicitly stated; assume minimum 2 years based on reference requirements
  • Certifications: BLS Certification required
  • References:
    • 2 supervisory references needed at submittal
    • 1 reference within the past 12 months
    • 1 reference within the past 3 years
  • Employment History:
    • Candidates must NOT have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit (including CHI and/or Dignity) within the past year
    • Candidates who were employed PRN through outside firms will be considered
  • Specific Requirement: Candidates must have recent experience as a Care Coordinator at a California Dignity Health Hospital; candidates without this experience will NOT be considered
  • Prior Travel Experience: Not specified or required

Job Details

  • Patient Load: 15-25 patients per day, potentially more depending on staffing
  • Floating: Will float to all units except surgery
  • Patient Population: Cardiac, surgical, oncology, critical care, etc.
  • Orientation:
    • One day orientation with an RNCC
    • 8-hour day shifts every other week (EOW)
    • First to float

This position is ideal for Care Coordinators with recent experience in California Dignity Health Hospitals who are comfortable managing a diverse patient population and floating across multiple hospital units.
