Job Opportunity: RN Step Down - Travel Position

Location: Cleveland, Ohio
Contract Length: 13 weeks


Position Overview

Required Certifications & Licenses

  • BLS (American Heart Association only)
  • ACLS (American Heart Association only)
  • NIHSS Certification
  • Valid permanent RN license (no temporary licenses accepted) with primary source verification within 30 days of start; must be updated every 2 years with license renewal

Required Health & Immunization Credentials

  • MMR: Positive titers within last 15 years; booster protocol if low titer
  • Varicella: Positive titers within last 15 years; booster protocol if low titer
  • Hepatitis B: Proof of full series, partial series, or signed declination (no childhood vaccines allowed); positive titers accepted within 15 years
  • COVID-19 Vaccine: Proof preferred but not required; medical or religious exemptions accepted
  • Influenza Vaccine: Required during Oct-March; no declinations allowed, exemptions require documentation
  • Tdap: Proof of vaccination within past 10 years; declinations not accepted
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Negative test within last 6 months; annual requirement; if positive, clear chest x-ray and TB questionnaire required
  • Fit Test: Completed within 1 year of start; specific mask models required
  • Physical/Health Statement: Pre-employment physical exam within 1 year stating ability to work without restrictions

Background & Verification Requirements

  • Criminal Background Check: Must be completed within 6 months prior to start; includes SSN trace, county searches, national criminal sweep, and NSO
  • Ohio Fingerprints: Required for Ohio residents (5+ years or less than 5 years with FBI fingerprint check); attestation allowed if pending with proof of completion
  • Social Security Trace: If not included in background check, must be completed within 1 year prior to start
  • National Database Criminal Background Check: For Ohio residents to meet fingerprint requirements
  • OIG Search: Completed within 6 months prior to start; initial hire only
  • SAM/EPLS/GSA/OFAC Screening: Completed within 6 months prior to start; initial hire only
  • Drug Screen: 10-panel drug screen within 60 days prior to start; instant drug screens not accepted; must include lab report and MRO if positive

Professional & Employment Verification

  • Education Verification: Highest level of education verified; international education requires equivalency documentation
  • Employment Verification: Verification of employment within last 5 years through third-party; attestations not allowed
  • UHS I-9 Attestation: Proof of eligibility to work in the U.S. required
  • Competency Exam: Must score at least 80%; completed within 1 year prior to start
  • Core Competencies: Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required; completed within 1 year prior to start
  • Skills Checklist: Clinical skill-specific checklist completed within 1 year prior to start (initial hire only)

Additional Requirements

  • Resume & References: Resume including education and work history, 2 professional references within last 2 years, and completed job application required
  • Badge Photo: Professional headshot (shoulders up, plain background, JPEG format)
  • Driver's License/State ID: Non-expired photo ID required
  • UHS Module Completion Certificate: Completion of safety and compliance training modules prior to clearance

This position requires strict adherence to credentialing, health, and compliance standards to ensure patient safety and regulatory compliance.
