Schedule: Weekend Nights, 3 shifts of 12 hours each (Friday, Saturday, Sunday, 5:30 PM - 6:00 AM)
Contract Length: 13 weeks

Required Credentials & Compliance

Personal Health History Requirements (Must be completed prior to onboarding)

• COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead (declination not accepted for CA or IL).
• MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
• Fit Test
• Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Hep B titers NOT accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead for flu season 10/1/2024 - 4/30/2025 and 10/1/2025 - 4/30/2026 (declination not accepted for CA or IL).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Positive TB requires chest x-ray within last year for MI, IN, CT, MA; within 5 years for other states.
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days), must include specific substances; no THC allowed.
• Trinity Color Vision Test

Professional Requirements (Must be completed prior to onboarding)

• Trinity Profile: Resume, skills checklist within a year, 2 professional references (required for submission).
• Confidentiality & Security Agreement: Signed Trinity Health confidentiality and system security agreement.
• Conflict of Interest Disclosure Form: Signed form on file.
• Criminal Background Check: Federal, nationwide, county checks from last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma verified by third party screening.
• Exclusion Lists Checks: EPLS, GSA/OIG, NSO reports dated within 30 days.
• Integrity and Audit Services Form: Signed shortly before start.
• Joint Commission Testing: CORE exam within the last year.
• System Access Form: Completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Specialty Specific Competency Testing: Specialty exam completed within the last year.
• Code of Conduct Form: Signed form on file.
• I9 Verification: Standard I9 completed by agency.
• License/Certificate Verification: Must be dated within 30 days of start; online verification required.

Certifications

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must include front and back images of card.

Summary
