Job Details

• Schedule: Every Third Weekend / End of Hour (EOH)

Required Experience

Certifications & Licenses (Must be current and valid)

• BLS (Basic Life Support) - American Heart Association certification required for all direct patient care personnel
• ACLS (Advanced Cardiovascular Life Support) - Required for RNs in critical care areas (CCU, ED, PACU, MICU, SICU)
• Valid RN License with online verification dated within 30 days of start

Health & Safety Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (includes cocaine, amphetamines, opiates, PCP, benzodiazepines, methadone, and others; no THC) completed within 30 days of start
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted in CA or IL)
• Influenza Vaccine (proof or declination/exemption with agency letterhead; declination not accepted in CA or IL)
• MMR (Measles, Mumps, Rubella) - proof of 2 vaccinations or IgG titer
• Hepatitis B vaccination series (at least first dose prior to start)
• Varicella vaccination or IgG titer proof
• 2 Step PPD or IGRA (Quantiferon/Tspot) for TB screening
• Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)
• Physical/Health Statement confirming fitness to work (exam within last year)
• Trinity Color Vision test

• Trinity Profile submission (resume, skills checklist within a year, 2 professional references)
• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed by agency)
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28 completed)
• Specialty Specific Competency Testing (copy of specialty exam within last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG Exclusion check dated within 30 days
• Joint Commission CORE Exam within last year
• NSO (National Sex Offender) check dated within 30 days (includes aliases and locations)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (required for Iowa)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)

Summary

This is a 13-week long-term RN assignment in Medical/Surgical and Telemetry units located in Mason City, Iowa.
