Job Opportunity: RN Medical / Surgical / Telemetry

Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• Every 3rd weekend
• Every other holiday

Required Certifications & Licenses

• BLS (American Heart Association certification required for all direct patient care personnel)
• ACLS (Required for RNs in CCU, ED, PACU, MICU, SICU)
• NIHSS
• Valid nursing license with verification dated within 30 days of start
• Fit Test
• TNCC (optional)

Required Health & Immunizations

• COVID-19 Vaccine (including Sept 2025 booster) or approved declination/exemption (declination not accepted in CA or IL)
• Influenza Vaccine or approved declination/exemption (declination not accepted in CA or IL) for flu season 10/1/2024 - 4/30/2025
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (proof of 1st vaccination prior to start; titers not accepted)
• Varicella (proof of 1 vaccination or IgG titer)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Tdap vaccine (declinations accepted if no pediatric interaction)
• Physical/Health Statement confirming fitness to work (exam within last year)
• Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)

Required Professional & Compliance Documents

• Trinity Code of Conduct Form (signed)
• Trinity I9 Form
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non Employee Upload Form (pages 1-3 and 28)
• Specialty Specific Competency Testing (copy of specialty exam within last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification, dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS Report (dated within 30 days)
• GSA/OIG Exclusion Report (dated within 30 days)
• Joint Commission CORE Exam (within last year)
• NSO Report (dated within 30 days, includes all aliases)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check, within 30 days prior to start)
• OFAC Sanctions Check (completed within 30 days of start)
