Description

Job Opportunity: RN Step Down (4W Nights)

Location: Evansville, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Unit & Patient Population

  • Dedicated Stroke Unit
  • 30 beds
  • Nurse to Patient Ratio: 1:4
  • Patient conditions include Medical, DKA, Cardiac, GI, Renal, Sepsis, Trauma, Surgery

Required Qualifications

Licensure

  • Current RN license in Indiana with primary source verification within 30 days prior to assignment start
  • Nursys verification required within 30 days prior to start
  • Driver’s License or State ID (must not be expired and address must match license requirements)

Certifications

  • BLS (AHA required)
  • ACLS (AHA required)
  • NIH Certification (Group A updated yearly; Groups B-F updated every 2 years)

Exams & Testing (Must be completed prior to start and maintained as required)

  • Ascension Pharmacology Exam (minimum 80%, max 2 attempts, updated every 3 years)
  • Ascension Medical Surgical Exam (minimum 80%, max 2 attempts, updated every 3 years)
  • Ascension EKG Exam (minimum 80%, max 2 attempts, updated every 3 years)
  • Ascension Core Competencies (HIPAA, Joint Commission, NPSG, OSHA topics; minimum 80%, max 2 attempts, updated every 3 years)
  • Ascension Skills Checklist (clinical only; completed within 1 year of start, updated annually)

Health & Immunizations

  • MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
  • Varicella: Documented proof of two vaccines, serological evidence, or verified history (no declinations accepted)
  • Tdap: Valid within 10 years (no declinations accepted)
  • Hepatitis B: Documentation of vaccination series completion or signed declination
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
  • Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually
  • 2 Step PPD or IGRA (Quantiferon/Tspot): Required with appropriate timing and annual TB questionnaire
  • Positive PPD requires negative chest X-ray within 1 year and annual TB questionnaire
  • Health Statement or physical exam prior to first assignment, updated annually
  • 12 Panel Drug Screen within 30 days prior to start, updated annually

Professional Requirements

  • Resume/Profile submission required
  • At least two professional references prior to first assignment
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and others within 30 days of assignment start; updated annually or after break in service > 90 days
  • EPLS check within 90 days of start; rerun if break in service > 90 days
  • Education verification relevant to clinical work within 90 days of start; rerun if break in service > 90 days
  • Employment verification for past 7 years; updated annually or after break in service > 90 days
  • Signed Assignment Agreement and Onboarding requirements completed onsite
  • Badge photo (forward facing headshot with white or blue background, no alterations)
