CT Technologist Position - Iowa, Dubuque

Job Details

• Job Title: CT Technologist
• Location: Dubuque, Iowa
• Employment Type: Long-Term Assignment (approximately 13 weeks)
• Shift: Varies between 06:00-18:30, 06:30-19:00, or 07:00-19:30
• On-Call: Every 4th weekend from 06:00-18:30
• Holiday Rotation: Every 4th holiday

Required Qualifications

• Valid CT Technologist license in Iowa
• Minimum of 1 year of CT Technologist experience within the last year
• BLS certification issued by the American Heart Association

Health and Safety Requirements

• Fit test
• Trinity 10 Panel Drug Screen (including specific substances; no THC)
• COVID-19 vaccine proof (including booster or approved declination/exemption)
• Influenza vaccine proof or declination/exemption
• MMR vaccination proof or IgG titer
• Hepatitis B vaccination series (proof of initiation required; titers not accepted)
• Varicella vaccination proof or IgG titer
• 2 Step PPD or IGRA (Quantiferon/Tspot) test; chest x-ray if positive TB as per regional requirements
• Physical/Health statement confirming fitness to work (exam within last year)
• Current, non-expired Tdap vaccine (declination accepted if no pediatric interaction)

Professional and Compliance Requirements

• Signed Code of Conduct form
• Signed Confidentiality and Information System Security Agreement
• Signed Integrity and Audit Services form
• Completed Non-Employee Upload form
• Conflict of Interest Disclosure form
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification; must be dated within 30 days of start)
• Education Verification (highest academic degree or equivalent)
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• Joint Commission CORE Exam within the last year
• NSO (National Sex Offender) check dated within 30 days, including aliases and locations
• System Access Form completed and uploaded one week before start
• Iowa-specific Child and Adult Dependent Abuse Training
• Iowa-specific Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check (within 30 days prior to start)
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days
