• Facility Type: Level 1 Trauma Hospital with 3 CT scanners (1 GE, 2 Siemens)

Required Qualifications

• Valid CT Technologist license in Indiana
• Minimum of 1 year of CT Technologist experience, including experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association (AHA)

Health and Safety Requirements

• Proof of immunizations or immunity for:

  • Measles, Mumps, Rubella (MMR) – two vaccines or positive titer; no declinations accepted.
  • Varicella – two vaccines, serological immunity, or documented history; no declinations accepted.
  • Tdap – valid within 10 years; no declinations accepted.
  • Hepatitis B – vaccination series completion or signed declination.
• Influenza vaccine is optional; if declined during flu season (Oct 1 – Mar 31), mask wearing is required.
• Tuberculosis screening:

  • 2-step PPD or Negative IGRA within specified timeframes.
  • If positive, must provide negative chest X-ray within one year and complete annual TB questionnaire.
• Fit testing for respiratory protection required annually; preferred before start but can be completed onsite.
• Health statement or physical exam confirming good health required prior to first assignment and updated annually.
• 12-panel drug screen within 30 days prior to start and annually thereafter; retesting required if break in service exceeds 90 days.

Onboarding and Documentation Requirements

• Submission of professional resume/profile and at least two professional references prior to assignment.
• Driver’s license or state ID must be current, show front and back, and address must comply with licensing rules.
• Completion of core competencies including HIPAA, Joint Commission, OSHA, and specialty-specific skills checklist within one year and updated annually.
• Criminal background check including Social Security trace, OIG search, and national sex offender registry within 30 days prior to start; updated annually or after breaks in service over 90 days.
• Education verification relevant to clinical work within 90 days prior to start; must be updated after breaks in service over 90 days.
• Signed assignment agreement and confirmation of assignment on agency letterhead required upon onboarding and for each extension.
• Badge photo with white or blue background required for onboarding.
• Completion of onboarding education attestation (6 hours) with traveler’s signature required.
• Employment verification covering the past seven years required; updated annually or after breaks in service over 90 days.

Additional Notes

• The Medical Imaging Contract Worker Form is completed onsite and does not need to be uploaded prior to start.
• Post-assignment hiring manager evaluation and legal name change documentation are optional.

This position is ideal for CT Technologists seeking a night shift role in a high-acuity trauma hospital setting with a 13-week contract duration. Compliance with detailed credentialing, health, and onboarding requirements is mandatory.
