Job Description

  • Provide direct nursing care following established policies, procedures, and protocols.
  • Implement and monitor patient care plans; record and communicate patient condition appropriately.
  • Coordinate care among all disciplines to ensure well-coordinated patient care.
  • Follow physician and nursing orders accurately.
  • Assess and coordinate patient discharge planning with the healthcare team.
  • Work requires critical thinking, decisive judgment, and ability to work with minimal supervision in a stressful environment.
  • Use personal protective equipment following Standard Precautions.

Required Qualifications

  • Valid RN license in Oklahoma
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum 2 years of Medical/Surgical nursing experience, with experience within the last year
  • Stroke experience, including proficiency with NIH Stroke Scale (NIHSS)
  • Successful completion of Ascension Medical Surgical Exam
  • Completion of Ascension Core Competencies

Onboarding and Compliance Requirements

  • Complete pharmacology and medical-surgical competency exams prior to start; minimum 80% score required, with only two attempts allowed.
  • Complete core competencies including HIPAA, Joint Commission, NPSG, and OSHA topics within one year prior to start; minimum 80% passing score required.
  • Complete unit-specific checklist (optional for onboarding).
  • Complete onboarding education attestation and nursing contract worker onboarding onsite.
  • Complete Cerner test-out and training checklist with signed attestation prior to start.
  • Provide a forward-facing badge photo with white or blue background (no alterations).
  • Submit documented proof of immunizations and health screenings:
    • MMR (two vaccines or positive titer; no declinations accepted).
    • Varicella (two vaccines, serological evidence, or verified history; no declinations accepted).
    • Tdap (valid within 10 years; no declinations accepted).
    • Hepatitis B vaccination series completion or signed declination.
    • Influenza vaccine (optional; if declined during flu season, mask required).
    • Tuberculosis screening: 2-step PPD or IGRA within required timeframes; if positive, chest X-ray and annual TB questionnaire required.
  • Fit test for respiratory protection required prior to or onsite at start; annual updates required.
  • Submit a statement of good health or completed physical exam prior to first assignment, updated annually.
  • Complete a 12-panel drug screen within 30 days prior to start and annually thereafter.
  • Provide professional references (minimum two) and employment verification for the past seven years.
  • Submit a current, unexpired driver’s license or state ID matching address on file.
  • Complete criminal background check including Social Security Trace, OIG Search, National Sex Offender, and other required screenings within 30 days prior to start; updated annually or after breaks in service longer than 90 days.
  • Education verification relevant to clinical work must be completed within 90 days prior to start; updated if break in service exceeds 90 days.
  • Provide signed assignment agreement and confirmation of assignment on agency letterhead with accurate start and end dates.

Additional Notes

  • Must be able to work in a stressful environment with minimal supervision.
  • Critical thinking and decisive judgment skills are essential.
  • Personal protective equipment use is mandatory.
