Required Qualifications

• Valid Radiology Technician license in Indiana
• Radiology Technician experience within the last year (minimum 1 year)
• ARRT (American Registry of Radiologic Technologists) certification
• BLS (Basic Life Support) certification issued by the American Heart Association

Experience

Onboarding Requirements

• State license verification dated within 30 days of start
• BLS certification card (front and back)
• Fit test
• Annual TB questionnaire and 2-step PPD or IGRA test (with chest x-ray if positive TB)
• Trinity 10-panel drug screen completed within 30 days of start (no THC)
• Proof of COVID-19 vaccination or approved exemption/declination (booster required by Sept 2025; declination not accepted in CA or IL)
• Proof of MMR vaccination (2 doses) or IgG titer
• Hepatitis B vaccination series started prior to start date
• Proof of Varicella vaccination or IgG titer
• Physical/health statement confirming fitness to work (exam within last year)
• Tdap vaccine (current, non-expired) or declination if no pediatric interaction

Professional and Compliance Requirements

• Resume and skills checklist (within one year)
• Two professional references
• Signed Code of Conduct form
• Completed I-9 form
• Confidentiality and system security agreement
• Integrity and audit services form (signed shortly before start)
• Joint Commission CORE exam within the last year
• Conflict of Interest Disclosure form
• Criminal background check (federal, nationwide, county for last 7 years) including SSN verification, dated within 30 days of start
• Education verification (highest academic degree or diploma/GED)
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days
• OFAC (Office of Foreign Assets Control) sanctions check dated within 30 days
• System access form completed and uploaded one week before start
• Skills checklist provided by agency for clinical contractors

Additional Notes

• Positive TB requires chest x-ray within the last year for Indiana
• Drug screen panel includes: cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, methadone (NO THC)
• Declinations for influenza vaccine accepted except in CA or IL (influenza vaccine is optional)

This position requires working weekend night shifts and compliance with extensive onboarding and credentialing requirements to ensure patient safety and regulatory adherence.
