Respiratory Therapist - Iowa (Davenport)
Job Details
- Employment Type: Long-Term Assignment (LTA)
- Contract Length: 13 weeks
- Shift: 6:00 PM – 6:30 AM, every other week (EOW) plus 1 holiday per rotation
Required Qualifications
- Valid Respiratory Therapist license in Iowa
- Minimum of 1 year of Respiratory Therapist experience within the last year
- BLS issued by American Heart Association
- ACLS certification
- EPLS (Excluded Parties List System) check
Onboarding and Compliance Requirements
- Fit Test
- COVID-19 Vaccine (including booster by Sept 2025) or approved exemption/declination (declination not accepted in CA or IL)
- MMR: Proof of 2 vaccinations or IgG titer lab report
- Hepatitis B: Must have started vaccination series prior to start; titers not accepted
- Influenza Vaccine: Optional but recommended (proof or declination accepted except in CA or IL)
- 2-Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB per regional requirements
- Varicella: Proof of vaccination or IgG titer lab report
- Physical/Health Statement: Documentation of fitness to work within the last year
- 10-Panel Drug Screen: Completed within 30 days of start, no THC allowed
Professional and Background Checks
- Criminal Background Check (Federal, Nationwide, County for last 7 years), including SSN verification, dated within 30 days of start
- Education Verification (highest academic degree or diploma/GED)
- EPLS (Excluded Parties List System) check dated within 30 days
- OIG (Office of Inspector General) exclusion check dated within 30 days
- NSO (National Sex Offender) check dated within 30 days, including aliases and state-specific checks for MI
- Iowa-specific Child and Adult Dependent Abuse Registry check within 30 days prior to start
- Conflict of Interest Disclosure Form
- Code of Conduct Form
- Confidentiality and System Security Agreement
- Integrity and Audit Services Form (signed shortly before start)
- Non-Employee Upload Form (pages 1-3 and 28)
- System Access Form (completed and uploaded one week before start)
Competency and Skills Verification
- Specialty Specific Competency Testing: Must be completed within the last year
- Skills Checklist: Provided by agency for clinical contractors
- Joint Commission CORE Exam: Completed within the last year
Additional Notes
- Resume, skills checklist (within one year), and two professional references required for submission
- Must complete and upload all required documentation into the credentialing system prior to start
- Work schedule includes every other week rotation and holiday coverage as assigned
