Respiratory Therapist - Iowa (Davenport)

Job Details

  • Employment Type: Long-Term Assignment (LTA)
  • Contract Length: 13 weeks
  • Shift: 6:00 PM – 6:30 AM, every other week (EOW) plus 1 holiday per rotation

Required Qualifications

  • Valid Respiratory Therapist license in Iowa
  • Minimum of 1 year of Respiratory Therapist experience within the last year
  • BLS issued by American Heart Association
  • ACLS certification
  • EPLS (Excluded Parties List System) check

Onboarding and Compliance Requirements

  • Fit Test
  • COVID-19 Vaccine (including booster by Sept 2025) or approved exemption/declination (declination not accepted in CA or IL)
  • MMR: Proof of 2 vaccinations or IgG titer lab report
  • Hepatitis B: Must have started vaccination series prior to start; titers not accepted
  • Influenza Vaccine: Optional but recommended (proof or declination accepted except in CA or IL)
  • 2-Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB per regional requirements
  • Varicella: Proof of vaccination or IgG titer lab report
  • Physical/Health Statement: Documentation of fitness to work within the last year
  • 10-Panel Drug Screen: Completed within 30 days of start, no THC allowed

Professional and Background Checks

  • Criminal Background Check (Federal, Nationwide, County for last 7 years), including SSN verification, dated within 30 days of start
  • Education Verification (highest academic degree or diploma/GED)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • OIG (Office of Inspector General) exclusion check dated within 30 days
  • NSO (National Sex Offender) check dated within 30 days, including aliases and state-specific checks for MI
  • Iowa-specific Child and Adult Dependent Abuse Registry check within 30 days prior to start
  • Conflict of Interest Disclosure Form
  • Code of Conduct Form
  • Confidentiality and System Security Agreement
  • Integrity and Audit Services Form (signed shortly before start)
  • Non-Employee Upload Form (pages 1-3 and 28)
  • System Access Form (completed and uploaded one week before start)

Competency and Skills Verification

  • Specialty Specific Competency Testing: Must be completed within the last year
  • Skills Checklist: Provided by agency for clinical contractors
  • Joint Commission CORE Exam: Completed within the last year

Additional Notes

  • Resume, skills checklist (within one year), and two professional references required for submission
  • Must complete and upload all required documentation into the credentialing system prior to start
  • Work schedule includes every other week rotation and holiday coverage as assigned
