Location: Wichita, Kansas
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Description
- Work in a 30-bed acute inpatient rehabilitation setting.
- Dress code: black scrubs preferred over business casual.
- Experience with neurological diagnoses is preferred.
- Shift: 7:00 AM – 3:30 PM.
Required Qualifications
- Valid Occupational Therapist license in Kansas.
- Minimum of 1 year of Occupational Therapist experience, including experience within the last year.
- Basic Life Support (BLS) certification issued by the American Heart Association.
Onboarding & Compliance Requirements
- Professional documentation:
- Resume/Profile.
- Two professional references.
- Signed assignment agreement (offer letter, initial confirmation, and extensions).
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 90 days of start; updated annually or after breaks in service.
- Employment verification for the past seven years; updated annually or after breaks in service.
- Education verification relevant to clinical work within 90 days of start; must be rerun after breaks in service.
- Skills checklist for specialty, completed within one year of start and updated annually.
- Confirmation of assignment on agency letterhead with accurate start/end dates.
- Personal health and immunization requirements:
- MMR: documented proof of two vaccines or positive IGG titer; no declinations accepted.
- Varicella: documented proof of two vaccines, serological immunity, or verified history; no declinations accepted.
- Hepatitis B: vaccination series completion or signed declination.
- Tdap: valid within 10 years; no declinations accepted.
- Influenza vaccine: optional; if declined during flu season (Oct 1 – Mar 31), mask wearing required.
- Tuberculosis screening: 2-step PPD or IGRA within specified timeframes; positive results require chest X-ray and annual TB questionnaire.
- Fit test for respiratory protection required prior to start or onsite (PAPR not done onsite).
- Health statement or physical exam prior to first assignment, updated annually.
- 12-panel drug screen within 30 days prior to start and annually thereafter; retesting required after breaks in service.
- Identification:
- Valid driver’s license or state ID matching address on file; must show front and back.
Additional Notes
- All onboarding documents and requirements must be completed and verified prior to or at onboarding as specified.
- No declinations accepted for certain vaccines and immunizations.
- Candidate must be prepared to comply with all health and safety protocols.
