Occupational Therapist (OT) - Home Health (Days)
Location: New Lenox, Illinois
Employment Type: Temporary
Shift: Days
Hours per Day: 8
Contract Length: 13 weeks
Job Summary
- Evaluate patient needs, establish goals, and provide interventions to help patients achieve independence with activities of daily living.
- Collaborate with physicians and other healthcare team members.
- Provide supervision to Occupational Therapy Assistants (OTAs) and OT/OTA students.
- Maintain a clean, safe environment and uphold patient privacy and confidentiality.
- Complete timely documentation in the electronic medical record.
- Develop professional skills through continuing education and evidence-based practice.
- Communicate effectively with patients, families, and healthcare professionals.
- Ability to transfer and position patients.
- Includes weekend rotation (Saturday and Sunday) as applicable.
Required Qualifications
- Valid Occupational Therapist license in Illinois
- Minimum 1 year of Occupational Therapist experience, including experience within the last year
- Experience in a Home Health setting
- Valid driver's license
- Basic Life Support (BLS) certification
