Physical Therapist – Outpatient Clinic

Location: Salida, Colorado
Employment Type: Contract (13 weeks)
Schedule: Monday - Friday, 40 hours per week

Job Description

• Provide physical therapy services within the scope of practice, focusing on Pelvic Health.
• Conduct thorough patient assessments to determine diagnosis, prognosis, and intervention strategies.
• Design and modify therapeutic plans to help patients regain daily functional abilities.
• Promote injury and disability prevention through health, fitness, and wellness education for all age groups.
• Participate in consultation, education, and research activities related to physical therapy.
• Maintain accurate and up-to-date patient records, including physician orders, treatment plans, progress notes, and discharge summaries.

Required Qualifications

• Valid Rehabilitation Therapist license in Colorado.
• 4 years of Rehabilitation Therapist experience.
• Rehabilitation Therapist experience within the last year.
• Pelvic Health Certification.
• CPR certification.
• 4 years of Pelvic Health experience.
• Physical Therapist experience.
