CT Technologist - Indiana (Mishawaka)

Job Details

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: Monday through Friday, 2:30 PM – 11:00 PM

Required Qualifications

• Valid CT Technologist license in Indiana
• Minimum of 1 year of CT Technologist experience within the last year
• ARRT (CT) certification
• BLS certification issued by the American Heart Association

Onboarding and Compliance Requirements

• Fit Test
• Annual TB Questionnaire and 2 Step PPD OR IGRA (Quantiferon/Tspot) testing (with chest x-ray if positive TB)
• Trinity 10 Panel Drug Screen (10-panel drug screen completed within 30 days of start; no THC)
• COVID-19 Vaccine proof (including booster by Sept 2025) or approved exemption/declination (declination not accepted for CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start (titers not accepted)
• Varicella: Proof of vaccination or IgG titer lab report
• Influenza Vaccine (optional): Proof of vaccine, declination, or exemption accepted except in CA or IL
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Trinity Color Vision and Latex Allergy Questionnaires

Professional and Background Checks (to be completed prior to or at onboarding)

• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification)
• Education Verification (highest academic degree or diploma/GED)
• EPLS (Excluded Parties List System) check
• OIG (Office of Inspector General) exclusion check
• NSO (National Sex Offender) check
• OFAC (Office of Foreign Assets Control) sanctions check
• Trinity Code of Conduct Form signed
• Trinity Conflict of Interest Disclosure Form signed
• Trinity Confidentiality and System Security Agreement signed
• Trinity Integrity and Audit Services Form signed shortly before start
• Trinity Joint Commission CORE Exam within the last year
• Trinity System Access Form completed and uploaded one week before start
• Trinity Orientation Guide completed
• Trinity Skills Checklist (provided by agency for clinical contractors)

Additional Notes

• Resume, skills checklist (within one year), and two professional references required for submission.
• License and certification verifications must be dated within 30 days of start.
• All required documents must be uploaded to the credentialing system as specified.

This position is suitable for CT Technologists licensed in Indiana with ARRT certification and BLS, able to work weekday evening shifts for a 13-week contract. Compliance with extensive health screenings, vaccinations, and professional background checks is mandatory.
