Job Opportunity: Physical Therapist - Inpatient Rehab
Location
- Thomasville, Georgia
Employment Type
- Travel/Contract
Assignment Details
- Start date: Approximately 3 weeks from offer
- Contract length: 13 weeks (with possible extension)
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM, plus 1 weekend per month (Every Other Weekend)
- Float position
- Minimal to no Return to Office (RTO) required
Required Qualifications
- Valid Physical Therapist license in Georgia
- Minimum of 1 year of Physical Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
- Valid Driver's License
- Experience with Cerner electronic medical record system
Clinical Skills and Experience
Experience with the following conditions and treatments is required or highly preferred:
- Body mechanics
- Cervicalgia and low back pain
- Post-operative care for cervical surgery, shoulder repair, spinal surgery
- Spinal conditions and spinal cord injury
- Total hip and knee replacement rehabilitation
- Cognitive disorders, Multiple Sclerosis, Muscular dystrophy, Parkinson’s disease
- Post-stroke rehabilitation
- Traumatic brain injury

Documentation and Compliance Requirements
Candidates must provide and maintain the following, updated as required:
Background and Verification
- 7-year criminal background check covering all locations lived/worked in the past 7 years
- Social Security Number trace, county checks, national sex offender registry, FACIS Level 3, nationwide checks
- Education verification (transcript or diploma accepted if education verification is not available)
- Employment application/resume
- Two recent supervisor/manager references verified within 12 months
- Performance and internal evaluations if applicable (for candidates with prior experience at the facility)
Licensure and Certifications
- Primary source verification of state license
- Annual competency exams and medication/pharmacology competency
- Annual OSHA, HIPAA, and Joint Commission training attestation or testing
- Physical exam within 1 year of start, updated annually
- Negative 10-panel drug screen within 30 days of start, updated annually
- Fit test for masks completed onsite at no cost
- Required immunizations or proof of immunity (or approved declination forms):
- Measles (Rubeola)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer, series, or declination)
- TB testing: QGold or TSPOT within 30 days of start (no PPDs accepted); if past positive, chest X-ray within 2 years and TB questionnaire within 30 days
- Flu vaccine required during flu season (September 3 through March 31) unless approved medical/religious exemption is provided; mask required if exempt
Facility-Specific Requirements
- Completion of assigned HealthStream modules prior to first day
- Submission of a passport-style photo for badge within 24 hours of signing
- Completion of facility-specific forms including IS115 and IS119 (time sensitive, wet or DocuSign signature required)
- Review of dress code policy
- Flu exemption forms if applicable
Additional Notes
- Candidates must reside more than 30 miles from the facility to be considered
- Qualivis submission form required with all submissions
- Documentation such as driver’s license must be a color copy (black and white not accepted)
- Fingerprinting requirements apply only to CNAs; not applicable for this PT role
This position offers a structured day shift with a focus on acute inpatient rehabilitation therapy, requiring a licensed Physical Therapist with acute care experience and strong clinical skills in neurological and orthopedic rehabilitation. Compliance with extensive background, health, and facility-specific documentation is mandatory.
