Job Overview

• Location: Cleveland, Ohio
• Shift: 7:00 AM - 7:30 PM
• Contract length: 13 weeks

Required Qualifications

• Active RN license
• Non-expired photo ID
• Resume with education and work history
• 2 professional references from the last 2 years
• Completed job application
• Employment history form confirming previous employment at the facility, if applicable

Onboarding / Compliance Requirements

• 2-step TB test or IGRA with negative reads within the last 6 months; annual requirement
• Background check within 6 months of start
• OIG search within 6 months prior to start
• SAM/EPLS/GSA/OFAC screening within 6 months prior to start
• Drug screen within 60 days of start
• Safety and compliance training modules must be completed before clearance
• UHS badge photo
• Influenza vaccine documentation is required from October through March; if declining, masking attestation and influenza declination forms are required, and if medically exempt, a medical accommodation form plus physician-signed letter are required

Additional Notes

• If the TB test is positive, a clear chest x-ray and current TB questionnaire are required
• If the background check has a hit, a candidate statement is required for review
• If the drug screen is positive, an MRO report is required
