RN Case Manager
Location: Nashville, Tennessee
Shift: 08:00 - 16:30
Contract length: 12 weeks
Role Summary
Coordinates, plans, and facilitates services for individuals by assessing needs, developing and updating care plans, monitoring progress, and collaborating with interdisciplinary teams.
Required Qualifications
- Valid RN license in Tennessee
- Case Management experience within the last year
- 1 year of Case Management experience
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License

- Core competencies completed within one year of start and updated every 3 years
- Onboarding education attestation
- Nursing contingent worker onboarding
- Nova StatStrip pre-hire module
- Cerner test-out
- MMR documentation
- Fit test
- Hepatitis B documentation
- Health statement or completed physical exam
- 2-step PPD or IGRA
- Varicella documentation
- Tdap documentation
- Influenza vaccine requirement may be waived with declination, with masking required during flu season
- HIPAA, Joint Commission, NPSG, and OSHA topics training
- Background screening items including SSN trace, OIG search, national sex offender search, county, national criminal, OFAC, and VSOP checks
