Description

Case Manager

Job Overview

  • Travel/contract role in Chesapeake, Virginia
  • Day shift: 5x8 hours, 8:00 AM–4:30 PM
  • Schedule includes weekdays, some weekends, and holidays
  • No overtime without supervisor approval
  • Longer assignments through January 9, 2027 are available but not required
  • Travelers must work two of the three winter holidays

Required Qualifications

  • Valid RN license in Virginia
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Case Management (5 years of experience)
  • Case Management experience within the last year
  • Experience working with Adult patients
  • Experience in an Acute Care setting
  • Discharge planning experience
  • Knowledge of CMS regulations
  • Utilization Review (InterQual)
  • Utilization Review experience
  • Experience with Epic
  • BLS issued by American Heart Association

Preferred Certifications

  • CCM (Certified Case Manager)
  • ACM (Accredited Case Manager)

Compliance / Submission Requirements

  • No license or background hits
  • Completed cover sheet
  • Current NURSYS report
  • All requested time off listed at submission
  • Proof of 2 references
  • Complete application/resume
  • Unit-specific skills checklist
  • Government-issued photo ID
  • Fingerprinting required
  • Physical exam within 12 months of start
  • Fit test for N95 mask within 1 year of start
  • Negative TB test or chest X-ray within 1 year of start
  • Negative 10-panel drug screen within 12 months of start
  • Flu vaccination or approved exemption
  • COVID vaccine documentation if available
  • State license verification
  • Orientation safety manual acknowledgement
  • Pass/fail testing acknowledgement
  • Pass/fail EKG/medication testing
  • Unit-specific job description form
  • Vehicle registration form
  • Traveler information grid
  • National criminal record request form
