Case Manager

Job Overview

Travel/contract role in Chesapeake, Virginia
Day shift: 5x8 hours, 8:00 AM–4:30 PM
Schedule includes weekdays, some weekends, and holidays
No overtime without supervisor approval
Longer assignments through January 9, 2027 are available but not required
Travelers must work two of the three winter holidays

Required Qualifications

Valid RN license in Virginia
Associate of Science in Nursing / Associate Degree in Nursing
Case Management (5 years of experience)
Case Management experience within the last year
Experience working with Adult patients
Experience in an Acute Care setting
Discharge planning experience
Knowledge of CMS regulations
Utilization Review (InterQual)
Utilization Review experience
Experience with Epic
BLS issued by American Heart Association

Preferred Certifications

CCM (Certified Case Manager)
ACM (Accredited Case Manager)

Compliance / Submission Requirements

No license or background hits
Completed cover sheet
Current NURSYS report
All requested time off listed at submission
Proof of 2 references
Complete application/resume
Unit-specific skills checklist
Government-issued photo ID
Fingerprinting required
Physical exam within 12 months of start
Fit test for N95 mask within 1 year of start
Negative TB test or chest X-ray within 1 year of start
Negative 10-panel drug screen within 12 months of start
Flu vaccination or approved exemption
COVID vaccine documentation if available
State license verification
Orientation safety manual acknowledgement
Pass/fail testing acknowledgement
Pass/fail EKG/medication testing
Unit-specific job description form
Vehicle registration form
Traveler information grid
National criminal record request form
