Care Coordination / Case Management

Job Overview

• Travel contract for an experienced care coordinator in a hospital setting near Sacramento, California.
• Schedule: 8-hour day shifts, every other weekend.

Required Qualifications

• Valid RN license in California
• Bachelor of Science in Nursing
• Case Management (2 years of experience)
• Case Management experience within the last year
• BLS
• Driver's License
• Utilization Management
• Discharge Planning

Preferred Qualifications

• Interqual experience

Work Setting / Responsibilities

• Case management and care coordination
• Works across all areas of the hospital
• No outpatient or home care coverage
• Handles discharge planning and utilization review
• Common duties include initial assessments and working with payers, uninsured, and homeless patients

• Business casual attire
• Hospitalist or intensivist in house
• Respiratory therapy available 24/7
• Pharmacy is not available 24/7
• Orientation includes 5 days of care coordination new employee orientation
• First to float
