Description

Job Description:
We are seeking a dedicated Registered Nurse (RN) Case Manager to coordinate and manage patient care. The ideal candidate will assess patient needs, develop care plans, coordinate with healthcare providers, monitor patient progress, and ensure quality and timely care.
Requirements:
Active RN license
Case management or clinical experience preferred
Strong communication and organizational skills
Ability to coordinate effectively with patients, families, and healthcare teams

Della Infotech Inc Job ID #30125. Pay package is based on 7 hour shifts and 36.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
