Description

40-hours per week (Dependent on operational need, may be 4x10's or 5x8's)
Day shift, Monday – Friday
Candidates have at least 2 years of case management experience.
6-months discharge planning Case Management experience in an Acute Care hospital. This should ideally be a large organization/teaching hospital with 500 beds or more
