Description

We are seeking an experienced Registered Nurse Case Manager to provide comprehensive case management and care coordination services in an acute-care hospital environment.
The RN Case Manager will be responsible for coordinating patient care, utilization review, discharge planning, developing plans of care, and collaborating with physicians, social services, transportation, and other members of the healthcare team.
CA RN License: Must be active and in hand at the time of submission.
Experience: 3+ years of acute-care Case Management experience – REQUIRED.
BLS: Required.
Epic: Highly preferred.
Flu Vaccine: Required during flu season with no exemptions.
COVID-19 Vaccine: Not required by the facility.
Weekend: Every other weekend.
