POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care coordination, discharge planning, and utilization review in an acute hospital setting.

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license; BLS; 1 year case management experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break; Discharge planning, utilization review, and Interqual experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M-F, 8-5, 40 hours; rotate weekends

SYSTEMS – Meditech, Midas, Careport

START DATE – ASAP
