Registered Nurse - Case Management (Travel)

Contract length: 12 weeks

Job Description

Coordinate management of patient care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
Enhance quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness.
Integrate case management, utilization review, and discharge planning functions.
Serve as a patient advocate addressing holistic patient needs.
Collaborate strongly with interdisciplinary teams (IDT).
Experience with MCG guidelines and discharge planning across acute care settings is required.

Schedule

Monday through Friday, 8:00 AM to 4:30 PM
40 hours per week

Required Qualifications

Valid RN license in Texas
Minimum 5 years of Case Management experience, including experience within the last year
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Experience in a Long Term Acute Care (LTAC) setting
Experience with MCG guidelines
Proficient in Discharge Planning
BLS certification issued by the American Heart Association

Prior travel experience is not explicitly required.
