POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Discharge Planning / Continuum of Care / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS – ME/compact RN License; BLS; 2 years recent case management experience in an acute hospital setting; Discharge planning and care coordination experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7:30 AM-4:00 PM

SYSTEMS – Epic
