POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination and Discharge Planning in addition to Utilization Review.

MINIMUM REQUIRED QUALIFICATIONS – ME/compact RN license; BLS; 2 years case management experience in an acute hospital setting

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7:30 AM-4:00 PM and every third weekend (7:30 AM-4:00 PM)

SYSTEMS – Epic

START DATE – ASAP
