POSITION SUMMARY – LCSW Social Worker

POSITION DUTIES – Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

MINIMUM REQUIRED QUALIFICATIONS – MA LCSW; 2+ years minimum of acute care hospital experience in social work

LENGTH OF ASSIGNMENT – 12-13 weeks

SHIFT / HOURS PER WEEK – 8a-4p

START DATE – ASAP
