POSITION SUMMARY – LCSW Social Worker

POSITION DUTIES – Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

MINIMUM REQUIRED QUALIFICATIONS –

• MA LCSW
• CANS cert
• AHA BLS
• Masters degree in Social Work
• 2+ years acute hospital experience in social work

PREFERRED QUALIFICATIONS – Epic experience; Charge Capability

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M-F 8:00a-4:30p
