StartDate: 8/17/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager – Home Health

POSITION DUTIES – coordinates and delivers skilled nursing care in patients’ homes, assesses their medical needs, develops and updates care plans, educates patients and caregivers, monitors progress, and collaborates with physicians and other healthcare providers to ensure safe and effective treatment.

MINIMUM REQUIRED QUALIFICATIONS – 

• VT/compact RN license
• BLS
• 1+ year recent home health and case management experience
• Oasis experience

PREFERRED QUALIFICATIONS – Experienced traveler

LENGTH OF ASSIGNMENT – 16 weeks

SHIFT / HOURS PER WEEK – Days, Tuesday - Saturday 

SYSTEMS – Oasis, Homecare Homebase HCHB

START DATE – ASAP
