Job Title: RN Case Manager – Home Health
Location: White River Junction, VT

Contract Length: 13 Weeks
Shift: Days – Full Time

Position Overview:

We are seeking an experienced RN Case Manager – Home Health to provide comprehensive nursing care and coordinate services for patients in the home setting. This position offers autonomy while working closely with an interdisciplinary care team.

Randolph, VT
Springfield, VT
White River Junction, VT
Lebanon, NH

Requirements:

2 years of Home Health experience – Required
Graduate of an accredited school of nursing

Active Vermont and New Hampshire RN licenses OR an unencumbered Compact RN License BLS – AHA Required
