StartDate: 8/31/2026 Pay Rate: $1600.00 - $2400.00

POSITION SUMMARY – LSW / LICSW Social Worker

POSITION DUTIES – Collaborates with patients, families, providers, and payers to assess care needs, coordinate services, facilitate safe and timely discharges, ensure appropriate resource utilization, and support revenue integrity through accurate documentation and compliance with coverage and reimbursement requirements.

MINIMUM REQUIRED QUALIFICATIONS –

• NH LSW / LICSW
• Social work degree
• BLS
• 2 years case management experience in an acute hospital setting
• Acute hospital experience during the past year with no more than a 90-day break
• Critical care experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8a-4:30p

SYSTEMS – Midas

START DATE – ASAP
