StartDate: 10/12/2026 Pay Rate: $1800.00 - $2500.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Supports safe and timely patient discharge transition (typically 14-18 patients). Coordinates DME and Post acute services. Identifies high-risk patients and discharge barriers, collaborate with interdisciplinary tea members, documents and coordinates care in Epic

MINIMUM REQUIRED QUALIFICATIONS –

• NYS RN license
• NYS PRI Certification
• Screen Certification
• BLS
• 2 years recent case management experience in an acute hospital setting

PREFERRED QUALIFICATIONS – Experience with Epic and Care Port transition

LENGTH OF ASSIGNMENT – 8 weeks

SHIFT / HOURS PER WEEK – 8:00am-4:00pm, 37.5hrs/week
