Patient Care Coordinator/RN Case Manager

POSITION SUMMARY Patient Care Coordinator/RN Case Manager

POSITION DUTIES 

• Strong discharge planning and care coordination experience in an inpatient hospital setting.
• Kaiser Permanente experience, particularly in utilization review and discharge planning workflows.
• Familiarity with post-acute placement processes, including SNF, home health, DME, transportation, and community resource coordination.
• Experience collaborating with multidisciplinary teams, physicians, nursing, social work, and external agencies to facilitate timely and safe discharges.

MINIMUM REQUIRED QUALIFICATIONS CA license

PREFERRED QUALIFICATIONS Kaiser experience

LENGTH OF ASSIGNMENT 13 weeks

SHIFT / HOURS PER WEEK Schedule: 8:00 AM to 4:30 PM

Week 1: Mon, Tue, Thur, Fri, Sat

Week 2: Sun, Mon, Wed, Thu, Fri

SYSTEMS 

• Strong Epic documentation and communication skills.
