Seeking an experienced Physical Therapist – Home Health with 2+ years experience in home health settings. 



Must be proficient with OASIS documentation and Homecare Homebase (HCHB). The PT will provide in-home evaluations, treatment plans, and patient-centered care, focusing on improving mobility and functional independence.



Must have a valid driver’s license and be comfortable traveling between patient homes. Optional weekend shifts available if desired.



Certifications required: Active PT License + AHA BLS/CPR (DPT required if licensed after Jan 2016)
